Many agencies quote Ukrainian surrogacy as a single “from” number. That hides the two things that actually decide your budget: which program tier you choose, and which add-ons apply to your medical situation. Militta offers two tiers in Ukraine in 2026: the Basic program at $41,500–$48,500 and the Guaranteed program at $57,300–$68,000.
This article explains what each tier covers, what is quoted separately, and how to compare offers so that the number you sign is the number you pay.
The program fee: Basic vs Guaranteed
The Basic program ($41,500–$48,500) covers the full journey with a defined number of embryo transfers:
- IVF cycle (ovarian stimulation and egg retrieval if using own eggs).
- Embryo creation by ICSI and embryo culture.
- Embryo transfer to a pre-screened surrogate.
- Surrogate compensation, monthly allowance and medical insurance for the pregnancy.
- Prenatal monitoring and ultrasounds.
- Delivery at a partner hospital.
- Notarised surrogacy contract, legal and notary fees.
- Birth certificate naming both intended parents.
The Guaranteed program ($57,300–$68,000) covers the same journey, but continues until a live birth. Repeat embryo transfers and, if necessary, a replacement surrogate are part of the program rather than extra invoices. It costs more upfront and removes the single biggest source of budget uncertainty.
Where you land within each range depends on your medical situation and is fixed in a written quote before you sign.
What is quoted separately
The following items are not in the program fee. They are priced individually, and only when they apply to your case:
- PGT-A genetic testing of embryos. Recommended over age 35 or after recurrent miscarriage.
- Donor eggs or donor sperm. See our egg donation page.
- Additional IVF cycle if the first does not produce viable embryos.
- Twin pregnancy, applied only if confirmed.
- Apostille and translations for embassy use.
- Your travel and accommodation. Most families make two trips to Ukraine, for IVF and around the birth.
How to de-risk the budget
- Always use an escrow-backed, milestone-based payment structure. Do not pay the entire program fee upfront.
- Ask for a written quote that names every cost category above and says which ones are already in the number. If an agency will not provide one, walk away.
- Ask what happens if the first transfer fails. On the Basic program further attempts are quoted separately; on the Guaranteed program they are included.
- Confirm how C-section and NICU are handled. Read the insurance policy, not just a summary.
- Keep a contingency for travel, paperwork and timing changes that are outside any agency's control.
